DfE model FSM & pupil premium registration form

The form will help schools and local authorities determine how many pupils will attract funding for universal infant free school meals and the pupil premium. DfE recommend that schools ask all parents and guardians, regardless of family income or circumstances, to complete the form when they enrol their child in the school. Schools should then share the completed form with their local authority so they can run the necessary checks and make sure the school receives the universal infant free school meal and pupil premium funding they are entitled to.
